Doctor Patient role play? Pre visit questionaire SEXUAL HEALTH QUESTIONNAIRE (FEMALE) Before seeing the Doctor, please read through this questionnaire, and answer as many questions as you can. If you're not sure about an answer, mark "Uncertain" and the Doctor will discuss your recollections with you later. Many questions will seem very intimate, even private. Please be assured that the Doctor really does need as much information about your sexual history as possible in order to be of professional help. Some women find it hard to answer some questions because of embarrassment and shyness. If that's the case for you, just tell the Doctor about your embarrassment or shyness, and he will find especially relaxing, soothing ways of helping you. SECTION 1 Adolescence and Maturation *How old were you when you first thought about your sexuality? Describe the thoughts and any dreams you can remember. *Did any of your thoughts or dreams involve giving in to a man, or men, and allowing them to do things to you that should be kept secret? Were there any thoughts or dreams that made you feel especially embarrassed, humiliated, or degraded? (As an adult you could recognize this as submissive behavior.) *Do you remember the first time you recall feeling sexual arousal, wetness in your vagina, and/or muscle spasm or clenching in your pelvic area? Describe in detail what you were thinking or dreaming about when this happened. *How old were you when you first masturbated? Describe your masturbation fantasies. Describe where and how you touched yourself, and whether you used any sex toys. Did you ever masturbate with other girls? With boys? *When was your first period, and how did you feel about it when your Mother or other adult told you about it? ___________________________________ SECTION 2 First Sexual Experiences *Try to recall your first sexual experiences. Describe how the experiences were for you -- remember most people find themselves terribly uncomfortable the first few times, and often don't have much pleasure. Don't be embarrassed, but do be honest in your answer. Specifics that are required are your approximate age with each experience you describe, whether your partner was a boy/man or girl/woman, and what specific act or acts did you try or succeed in completing (eg -- oral sex performed on your partner, vaginal intercourse, anal intercourse, other) Provide at least two of your recollections, and do the best you can. Many times people are remembering events 30 or 40 years ago. SECTION 3 Experiences with Earlier Gynecological Exams *Many women report embarrassment during gynecological exams, because they know the Doctor will see that they are wet and obviously aroused. Some women have reported that they experience orgasms during their exams. Describe an early experience of disrobing for the exam, and then lying on the table, naked, and with your feet in the stirrups. Did it feel humiliating or embarrassing? Did it feel especially sexual? Did you feel proud and happy to be showing your "stuff" to the (male) Doctor? Feel free to write anything that's relevant to your early exams. SECTION 4 Current Sexual Activity and Practices Describe for the Doctor all sexual activity and practice that you are engaged in on an occasional or regular basis. This will include regular sexual intercourse between you and a man, as well as any deviant or unusual sexual practices (such as spanking or bondage before or during sex, , sexual contact that includes age play (pretending to be a young girl or baby), enforced chastity before sex, having your partner<b> nurse </font></b>at your breasts (if you are lactating), and so on. The Doctor will want explicit details in order to have a full picture of your sexual health. SECTION 5 Sexual Fantasies Everyone has sexual fantasies of one sort or another. Some fantasies are vague, while others are extraordinarily detailed and specific. Fantasies can occur any time -- while preparing a meal, while bathing or showering, while getting ready for bed, while using the toilet, and while engaged in love-making or masturbation. Since the Doctor is a participant in research on human sexual response, provide as much detail as you can. SECTION 6 Things You Want to Ask the Doctor Are there any questions you've always wanted to ask your OB/Gyn Doctor, but were afraid or too embarrassed to ask? Now is the time. |
